Warranty Policy

One year from the date of shipment when the products are properly installed and operated. The time period in which a purchased product may be returned or exchanged; may be extended through extended warranty plans purchased from the retailer or the manufacturer.



Dead on arrival (DOA), also dead in the field and brought in dead (BID), is a term used to indicate that a product was found to be already dead upon the arrival of professional technical check and assistance.

Failures of VPON products reported within thirty days after the shipment will be replaced with the new one. Under this condition, FORMOSA Inc. is liable for the shipping fees back and forth.

For the fast process of returning a product please make sure you fill the correct information in order to receive a refund, replacement, or repair during the product's warranty period.

Failures of VPON products reported after thirty days of shipment but within one year will be repaired and sent back to DISTRIBUTOR. Freight fee occurs will be paid by the corresponding party separately.

The purchaser of the product must contact the manufacturer (or distributor or retailer) to obtain authorization to return the product. The resulting RMA number must be displayed on or included in the returned product's packaging. Please keep in mind that no returns are accepted without this number.

FORMOSA DOES NOT WARRANT ANY COMBINATION OF OPERATIONS WHICH MAY BE SELECTED BY DISTRIBUTOR OR ITS RESELLER. THAT ARE OUTSIDE THE SCOPE OF VPON PRODUCT ITSELF.
